Our communities function based on state specific guidelines which regulate higher staffing ratios and resident centered activities. Memory Care residents are seniors that have advanced stages of Alzheimer's or other forms of dementia. By moving into a more structured environment with personalized care, we can help your loved one regain a sense of control and accomplishment. We help our residents lead a purpose filled life with meaningful connections. Residents whereabouts are monitored with gentle guidance provided-redirecting and affirming them based on their behavioral messages. Space is provided for wandering within a monitored environment

1. 24 hour staff with need specific training.
2. Staff Oversight by licensed nurse.
3. 24 hour monitoring.
4. Medication management based on level of need.
5. Reminders and escorts to meals and activities.
6. Light Housekeeping with attention to infection control-based on care plan.
7. Assistance with customized Senior centered activities
8. Grooming, Bathing, Dressing.
9. Coordinating efforts of the family/medical care team which may include PCP, Pharmacy, Home Health, Hospice, Durable medical equipment, Dialysis.

Home Care

Let us help you or your loved one stay as independent as possible.

With care ranging from 3 hours to 24X7, our highly trained staff can assist you with:

1. Medication assistance and reminders
2. Light housekeeping
3. Cooking and meal preparation
4. Errands
5. Assistance with Activities of Daily Living (ADL's) such as:

• Grooming
• Bathing
• Dressing

1. Coordination with outside Healthcare Professionals:

• Doctor Visits
• Hospice
• Home Health
• Physical Therapy

1. Home Care gives the patient as well as the family a lot of flexibility. Fees are based on hourly, daily or weekly rates.

Personal Care

Relevant care is personalized based on the resident care plan which is updated as needed. Personal Care with attendants enables them to maintain their treasured lifestyle. Many still drive, go out to worship, and enjoy entertaining guests. Personal Care enables them to maintain purpose in life. Your loved one has begun to lose some independence in areas of food preparation, managing medication, personal grooming and hygiene, but they are still thriving in social settings. Perhaps the risk of falls has drawn your attention
